
Reablement care is a short-term, outcome-focused home care service designed to help you regain independence after a hospital stay, illness, fall or loss of confidence. Rather than “doing for”, reablement works by supporting you to do things for yourself again — rebuilding daily living skills, mobility and routine so you can live safely and comfortably at home. In the UK this approach is sometimes called intermediate care or after-hospital care.
From your first conversation with Tidal Living, we focus on what matters to you. Our care professional will complete a person-centred assessment in your home and agree clear, time-bound goals — for example preparing a light meal, showering safely, getting up the stairs, or going out confidently. Your reablement care plan is then delivered through visiting care (domiciliary care) at the right frequency for you, with each visit designed to build strength, skills and confidence rather than create long-term dependence. Where appropriate, we’ll liaise with NHS therapists (e.g., occupational therapy or physiotherapy) to ensure your programme is joined-up and practical.
Reablement is time-limited. National guidance indicates it is typically up to six weeks when arranged by a local authority, though the exact length varies by individual needs and local arrangements. Private reablement packages can be shorter or longer depending on your goals.
Throughout your reablement we track progress against your goals, adapt visits as you improve, and step down support appropriately so you’re not paying for care you no longer need.
